A reading workflow for the Bayamón commuter corridor

Bayamón is the western terminus of the Tren Urbano, and much of its daily movement runs along the same axis: PR-22 and PR-2 toward the coast, PR-167 south toward Naranjito, the train and the expressways all pointed at San Juan. An imaging site here tends to serve patients who work in the metro and referring offices that sit in a different municipality from the scanner. That is a routing problem before it is a radiology problem.

A hub that is also a corridor

Bayamón is somewhere people pass through as much as somewhere they stop, which shapes who ends up holding the report.

A patient may live in Toa Alta, be imaged in Bayamón on the way home, and see a physician in Hato Rey the following week. That sequence is ordinary here. It also means the acquisition site and the ordering office frequently belong to different organizations, on different systems, with no shared record between them.

Planning for that starts by dropping the assumption that geography implies a relationship. The report follows the order and the authorization, not the address of the scanner. Everything else in the workflow is downstream of getting that one thing right.

Several scanners, one worklist, one recurring failure

When a network contains similar equipment, the first thing to break is knowing where a study came from.

Without that inventory, a mismatched study becomes a group chat. With it, the exception has an owner before it happens.

  • Give every sending device a distinct, documented identity.
  • Record which modalities and study types each device is expected to send.
  • Define what a complete study looks like for each of them.
  • Assign one named person per site who can correct demographics or complete an exam.

The report is a separate delivery problem

A successful image transfer proves that images moved. It proves nothing about the report.

Bayamón sites often deliver to a mix of nearby offices and metro practices, which means several delivery methods can coexist: an interface here, a portal there, a printed copy for a walk-in. Each of those routes needs its own test and its own failure behavior. The critical-result channel is the one that cannot be inferred from any of them, because it needs a person who answers, a way to confirm the message landed, and a written record that it did.

What to bring to a first conversation

A diagram of image sources and report destinations does more work than a description of the organization.

If several west-metro locations share a system, say so plainly. If the operating window shifts on weekends, say that too. Those details tell an integration team where the testing effort belongs. What they do not require is a single patient identifier, image, or clinical narrative, and none of those should ever be sent through a public contact form.

Common questions

Why does a Bayamón page keep mentioning other municipalities?

Because the referring offices, the staff, and the report recipients often are in other municipalities. A workflow drawn on municipal lines would be wrong on the first day.

Can images be sent from more than one location?

Potentially, but each sender and each report destination has to be identified, authorized, configured, and tested. There is no shortcut in which one connection covers several sites by default.

Does the website receive DICOM studies?

No. The public website is not a clinical system. It must not receive studies, reports, orders, or patient identifiers under any circumstances.

Who corrects an incomplete study?

The operating workflow names a local acquisition contact and a reading-side contact. Which one acts depends on whether the problem is acquisition, transmission, or interpretation.

How do reports reach a referring office in another town?

Through the delivery route established with the facility during onboarding. This page assumes no particular record system, portal, fax line, or interface.

Does the Tren Urbano matter to an imaging workflow?

Only as a sign of how the catchment behaves. It tells you that patients and staff commute along a corridor, so the people who need the report are spread along it too.

What can be shared safely through the contact page?

Facility type, municipality, modalities, systems, and what the workflow needs to accomplish. Not names, dates of birth, images, accession numbers, or clinical narratives.
